How to Apply for a Home Care Package

Understanding Home Care Packages

A Home Care Package (HCP) is a government-funded program that provides coordinated support for older Australians who need assistance with daily living activities. These packages are tailored to each individual’s needs, allowing them to continue living safely and comfortably at home.

There are four levels of HCPs, each offering a different amount of funding and support:

Services covered by HCPs can include:

  • Personal care (e.g., bathing, dressing)
  • Domestic assistance (e.g., cleaning, cooking)
  • Meal preparation
  • Nursing care
  • Mobility aids and home modifications
  • Social support and companionship
  • Transport assistance for medical appointments and shopping

 

Eligibility Criteria for Home Care Packages

To qualify for a Home Care Package, you must meet certain age and residency requirements.

  • Age requirement: Generally, applicants must be 65 years or older (or 50 years or older for Aboriginal and Torres Strait Islander people).
  • Residency requirement: You must be an Australian citizen, permanent resident, or hold an eligible visa.
  • Care needs: Your ability to manage daily activities independently is assessed to determine your eligibility.

Steps to Apply for a Home Care Package

Step 1: Register with My Aged Care

To begin to apply for a home care package, you will need to register with My Aged Care, the Australian Government’s portal for aged care services. You can register:

  • Online: Visit the My Aged Care website and follow the prompts.
  • By phone: Call 1800 200 422 and speak with an aged care representative.

During registration, you will need to provide basic details, including:

  • Personal information (name, date of birth, Medicare number)
  • Current living situation
  • Health conditions and mobility challenges
  • Any existing support services you receive

 

Step 2: Undergo an ACAT Assessment

Once registered, you will need to apply for an Aged Care Assessment Team (ACAT) assessment. This is an in-person evaluation, usually conducted at your home.

During the ACAT assessment, they will:

  • Ask about your daily activities and challenges
  • Evaluate your mobility, health, and cognitive function
  • Determine the level of care you require
  • Answer any questions you may have about Home Care Packages

You can have a family member or caregiver present during the assessment for support.

 

Step 3: Receive Your Assessment Outcome

After your ACAT assessment, you will receive a letter from My Aged Care outlining:

  • Whether you are approved for a Home Care Package
  • The level of care assigned to you (Level 1 to Level 4)
  • A unique referral code, which you will need to give to your chosen provider

Approval does not mean immediate access to services—there is a waiting list for Home Care Packages.

 

Step 4: Select a Home Care Provider

Once approved, you will need to choose an approved Home Care Package provider. When selecting a provider, consider:

  • The range of services they offer
  • Their reputation and experience
  • Any additional fees they charge
  • How well they align with your care needs and preferences

Step 5: Develop a Care Plan and Sign Agreements

After selecting a provider, you will work with them to develop a personalised care plan. This plan outlines:

  • The specific services you will receive
  • How your Home Care Package funds will be used
  • Your rights and responsibilities as a care recipient

Once finalised, you will sign a Home Care Agreement, which formalises the arrangement between you and your provider.

 

 

What to Expect After Applying

There may be a waiting period before your Home Care Package funding is allocated. The wait time will depend on the level of care you have been assigned and the demand for HCPs in your area. You can check your status by logging into your My Aged Care account or calling their support line.

Costs Associated with Home Care Packages

While the Australian Government subsidises Home Care Packages, there may be some out-of-pocket costs depending on your financial situation. Each package level has a set amount of annual government funding:

  • Level 1: Approx. $10,000 per year
  • Level 2: Approx. $18,000 per year
  • Level 3: Approx. $40,000 per year
  • Level 4: Approx. $60,000 per year

Depending on your income, you may be required to contribute:

  • Basic daily fee (set by the government; AMCS does not charge this fee)
  • Income-tested care fee (applies to individuals with higher incomes)
  • Additional services fee (for extra services beyond the funded amount)
